(Please print out, sign and return to the driver on your reservation date. All students must sign this form below. In the event that you do not have a copy of this form at your pickup location, your chauffeur will have a copy for all attendees to sign.)

My Chauffeur Limousine Services has adopted a set of guidelines to ensure the safety of all concerned. My Chauffeur has always had successful/safe proms (& Winter Formals) and our intention is to keep it that way. Please read over the guidelines ('rules') carefully. Fill in information needed and return with the signed contract.

Customer/student agrees that any of his/her activities or behavior deemed by the chauffeur to be illegal (intoxication, possession of alcohol, possession of drugs or drug paraphernalia etc.) or perilous will result in immediate termination of the job with no refund. Your chauffeur has the right to search belongings inside the vehicle and request to view inside pockets in clothing to search for drugs, alcohol or paraphernalia. Anything illegal to be found or suspected to be found will result in immediate termination of the job with no refund. Any illegal activity, significant arguments or fighting amongst or between the students or any students/persons or chauffeur will be dealt with by contacting the police. By order, the chauffeur is not allowed to intervene and will not intervene in any significant arguments/fighting between students or any other persons.
No body part is allowed outside the vehicle when the vehicle is moving. If the chauffeur deems it necessary for safety, the sun/moon roof or windows will be locked closed. This rule is strictly enforced.
If the chauffeur deems it necessary for safety, the privacy divider will be locked open.
Itinerary needs to be presented to My Chauffeur prior to reservation commencement.

All passengers must be approved on a manifest by the billing adult (Strictly applied). No student will be allowed in the vehicle at any time during the reservation agreement without the adult’s/parent's, legal guardian or billing client's permission. You cannot mix/match students from one limo to another or add students to the limo. Additionally, all students must be accounted for during the entire reservation agreement.

The adult/adult student purchasing the contract or the contact parent must approve any significant changes in itinerary after departure. The chauffeur will call the contact parent or purchaser if passengers request a significant change in itinerary. Additionally, all changes in stops or itinerary are not guaranteed and are subject to My Chauffeur approval. Significant changes will be denied without parent and chauffeur approval. The purchasing adult or contact person may, if desired, give prior approval, in writing, to the chauffeur, explaining which itinerary changes may or may not be made during the length of the contract. The chauffeur is always the final authority; the final decision/authority in all matters rests solely with My Chauffeur and the chauffeur.

In exchange, My Chauffeur and the chauffeur agree to be respectful of each student and their needs at all times and to maintain the safety of the group. In addition, we agree, for dedicated hourly reservations only, not to double book your time slot or switch your vehicle. For all reservations; we agree to be dedicated to your group and your group alone during the agreed upon time period ensuring your event is a success.

The chauffeur will provide his/her cell phone number to the contract purchaser or contact parent.
A contact parent’s name along with their phone number MUST be provided to the chauffeur. Contact parent needs to be available by phone for the length of the contract.

This form needs to be signed by purchasing adult, contact parent and initialed by each student and be returned along with reservation contract and deposit.

Termination of this contract will occur in the event of ANY serious safety violation, breach of contract or defiance of the rules set forth above which involves the safety of the students or chauffeur. The chauffeur HAS THE OPTION of contacting the contact parent before any termination of a reservation.

This form needs to be signed by purchasing adult, contact parent and each student and returned along with reservation contract and deposit.

A contact parent’s name along with their phone number MUST be provided to the chauffeur. Contact parent needs to be available by phone for the length of the contract. In the event of a serious safety violation or breach of contract which involves the safety of the students or chauffeur, the chauffeur HAS THE OPTION of contacting the contact parent before any termination of a reservation. Contact parent may need to provide transportation for individual students or the students as a group.

I HAVE READ AND AGREE TO THE FOLLOWING TERMS AND CONDITIONS: (all terms/conditions may not apply)

STUDENT AND CREDIT CARD HOLDER ACCEPTS FULL RESPONSIBILITY FOR CUSTOMER CAUSED VEHICLE DAMAGE OR SPECIAL CLEANING, AND AUTHORIZES COMPANY TO CHARGE FOR THE EXPENSES REGARDLESS AND IN ADDITION TO ANY PRIOR UNDERSTANDING AND AGREEMENTS. MY CHAUFFEUR IS NOT RESPONSIBLE FOR LOST OR STOLEN PERSONAL ITEMS LEFT IN THE VEHICLE EITHER DURING OR AFTER THE RENTAL PERIOD.

DUE TO THE NATURE OF THE 14 PASSENGER LIMO BUS (IF APPLICABLE), AND THAT IT IS A BUS, DESIGNED TO ALLOW STANDING, ALL PASSENGERS ARE AWARE AND DECLARE THAT THEY HAVE BEEN INSTRUCTED BY MY CHAUFFEUR TO REMAIN SEATED AT ANY TIME THE VEHICLE IS IN MOTION.
ALL PASSENGER’S MUST INITIAL THIS DOCUMENT THAT MY CHAUFFEUR HAS INSTRUCTED THEM THAT DUE TO THE NATURE OF THE 14 PASSENGER LIMO BUS, AND THAT IT IS A BUS, THAT ALLOWS STANDING THAT EACH PASSENGER “RIDES AT THEIR OWN RISK”. MY CHAUFFEUR WILL NOT BE RESPONSIBLE FOR ANY PASSENGER THAT INJURES HIS OR HERSELF IN ANY WAY WHILE ABOARD THE VEHICLE OR DURING THE RENTAL PERIOD. MY CHAUFFEUR WILL NOT BE RESPONSIBLE FOR INJURIES SUSTAINED TO ANY PASSENGER, INFLICTED BY ANOTHER PASSENGER EITHER ACCIDENTALLY OR PURPOSEFULLY.

BY INITIALING BELOW, I RELEASE AND FOREVER DISCHARGE MY CHAUFFEUR COMPANY, AFFILIATES AND ADMINISTRATORS AND ASSIGNS FROM ALL MANNER OF ACTIONS, CAUSES OF ACTIONS, DEBTS, ACCOUNTS, BONDS, CONTRACTS, CLAIMS AND DEMANDS FOR OR BY REASON OF ANY DAMAGE, LOSS OR INJURY TO PERSON AND PROPERTY WHICH HAS BEEN OR MAY BE SUSTAINED AS A CONSEQUENCE OF THE FOLLOWING:

THE CLAIM OR DISPUTE AROSE AS A RESULT OF DAMAGES OR INJURIES CAUSED AS A RESULT OF DRUGS AND/ OR ALCOHOL CONSUMPTION IN MY CHAUFFEUR TRANSPORTATION COMPANY AND AFFILIATES VEHICLES.
THE CLAIM OR DISPUTE AROSE AS A RESULT OF INJURIES OR ILLNESS CAUSED AS A RESULT OF THE USE OF MY CHAUFFEUR TRANSPORTATION COMPANY AND AFFILIATES VEHICLES.

IN THE EVENT THAT ANY PART OF THIS CONTRACT IS DECLARED UNENFORCEABLE, THE REMAINING PROVISIONS SHALL REMAIN IN FULL FORCE AND EFFECT.

DAMAGES TO THE FOLLOWING WILL BE PAID FOR IMMEDIATELY BY CHARGING THE FOLLOWING AMOUNTS TO THE CUSTOMER’S RENTAL AGREEMENT:

ANYTHING ELSE WILL BE ESTIMATED BY A PROFESSIONAL PERSON
* Stereo system comes with a remote control. If you fail to use the remote control system, touch the stereo system and damage it; you are liable for these damages. Similarly; if you play the music too loudly and damage any speakers, you are liable for these damages.

UNACCOMPANIED MINOR/SPECIAL NEEDS RELEASE FORM

Part 1: PARENT OR GUARDIAN – PLEASE NOTE:

1. The passenger(s) will be getting out of the vehicle by themselves.

2. My Chauffeur Transportation Co. does not assume any other responsibility for the unaccompanied minor/special needs passenger(s) after providing service between the pick-up and drop-out off locations.

3. This release needs to be valid for either a single date or a range of dates.

Part 2: AUTHORIZATION:

The minor/special needs persons will be getting out of the vehicle by him/herself at the destinations described in the 'Reservation Form - Contract'. I do give My Chauffeur Transportation Co. authorization to provide transportation between the (departure locations) and (arrival locations) described in the 'Reservation Form - Contract'.

I understand that My Chauffeur Transportation Co. is not acting as a guardian for the passenger(s) while on board the vehicle nor once the passengers leave the vehicle, whether on route or at the final destination.

My Chauffeur Transportation Co. is responsible for the transportation of the passenger(s) only and I authorize that the instructions specified above are valid for the date specified on the 'Reservation Form - Contract'.
